The May 2026 HIPAA Mandate: Why Physical Security Is No Longer Optional

Beginning May 2026, the updated HIPAA Security Rule will turn many formerly “addressable” safeguards into hard requirements. For small and mid-sized practices, that means physical security can no longer be treated as a best practice, it becomes a non-negotiable compliance obligation.

The new rule set emphasizes documented, repeatable proof that your facility is controlling who can access systems, records, and protected health information. That includes physical access controls, visitor management, secure equipment placement, and environmental safeguards such as power, temperature, and water intrusion protections.

Health and Human Service Office of Civil Rights (HHS OCR) also now expects these controls to be evaluated and validated by a qualified, unbiased professional with deep technical expertise. Internal self-checks or generic IT audits are no longer enough. Regulators want objective evidence that your controls are designed, implemented, and tested to a defensible standard. OCR is also steadily increasing the frequency and depth of HIPAA security audits, putting more practices under direct scrutiny each year.

For practice owners, the risk of treating this as a “checkbox” exercise is significant. OCR has made clear that superficial documentation without real, tested controls will not withstand scrutiny. Practices should plan for proactive, annual testing and continuous improvement to reduce the likelihood of findings, fines, and reputational damage.
